LOCATION: UK, LONDON, HANGER LANE GYRATORY (JUNCTION OF A406 (N.CIRC) AND A40
DESCRIPTION: A SILVER MERCEDES BENZ (REGISTRATION UNRECORDED OR PHOTOGRAPHED) WAS DETECTED AS A POSSIBLE THREAT BY LTC TACHIKOMA WHO WAS EN ROUTE VIA BICYCLE TO A MEETING. SHE AVOIDED SERIOUS INJURY BUT WAS NEVERTHELESS STRUCK AND FELLED BY THE FRONT NEAR SIDE OF THE VEHICLE WHICH THEN PROCEEDED TO ATTEMPT TO REVERSE OVER HER BODY.
TACHIKOMA EVADED AND WAS PREPARING TO DRAW HER SIDEARM AND ENGAGE THE ASSAILENT (NO DESCRIPTION) WHEN THE VEHICLE ACCELORATED AWAY USING THE TRAFFIC AS COVER. AN ARMED RESPONSE TEAM SUMMONED MEDICAL ATTENTION AND CLEARED AND SECURED HER WEAPON. TACHIKOMA WAS EVACUATED TO THE ROYAL NAVY HOSPITAL HASLAR (GOSPORT) FOR INITIAL (SECURE) TREATMENT, AND TRANSFERED TO DEPARTMENT 51 DSTL, PORTON DOWN ON 03 AUG 08 FOR OBSERVATION, AND REDESIGNATED AS AN OUT-PATIENT ON 04 AUG 08. THE PATIENT AND HER PARTNER SSGT KNIGHT. A. 100129, WERE THEN TAKEN IN TO PROTECTIVE CUSTODY BY E.R. PENDING INVESTIGATION AND RELOCATION.

DIAGNOSIS

TACHIKOMA SUFFERED BLUNT TRAMA TO HER LIMBS AND RIGHT HAND SIDE OF HER BODY LEADING TO ACUTE LACERATIONS, CONTUSIONS, STRAINS BUT NO SPRAINS. MILD INJURIES WERE SUSTAINED TO THE RIGHT GREAT TROCANTER, LEFT AND RIGHT ILIAC CREST, LEFT AND RIGHT PATELLA, LEFT TIBIA, RIGHT FIBULA, LEFT AND RIGHT TALUS, AND THE RIGHT TARSALS AND METATARSALS. AND FINALLY TO THE RIGHT HUMERUS AND OLECRANON PROCESS. MILD AND MODERATE STRAINS WERE SUSTAINED ON HER RIGHT SIDE TO THE STERNOMASTOID, TRAPEZIUS, DELTOID, SERRATUS, LATISSIMUS DORSI, EXTERNAL OBLIQUE, RECTUS FEMORIS, AND ON HER LEFT TO THE VASTUS LATERALIS, VASTUS MEDIALIS SHORT AND LONG PERONEUS, AND BOTH THE LEFT AND RIGHT PERONEUS TERTIUS. A SEVERE STRAIN RUPTURING THE GASTROCNEMIUS SOLEUS REQUIRED IMMEDIATE SURGERY (SUCCESSFUL).
IN ADDITION TACHIKOMA SUFFERED SERIOUS INFLAMATION TO THE FASCIA THROUGHOUT HER MUSCULATURE.

TREATMENT & PROGNOSIS: SATISFACTORY. ON GOING AS OUT-PATIENT.
FULL RECOVERY EXPECTED WITHIN 18 MONTHS OF INCIDENT, RELEASED FOR ACTIVE SERVICE IE LIGHT DUTIES AND NON COMBAT MISSIONS ONLY.

Tachikoma served as the RGHQ-51 XO (Executive Officer), from 15 JUN 07 through to CON-74 14 NOV 08, when she was appointed to replace LTC David Rhys as the RGHQ-51 CO (Commanding Officer). She was in charge of all off-world operations, through until CON-87 08-09 DEC 12 when she went in to the Reserve.

Without said action she and the remainder of her SG Team, would have been lost off-world. In recognition of this outstanding effort, she, is this day the 17th of February 2007, awarded the Achievement Medal."
